Annotation:

The article presents a decade of experience in performing liver transplantation from a cadaver donor and a related donor in the conditions of one center. The main stages of the development of a liver transplanta¬tion program in the region. Describes the reasons for which the liver transplantation was performed to patients. Describes the age-sex composition and the severity of the patients at the time of the operation. Ten-year experience of using various schemes of immunosuppressive therapy in recipients, an algorithm for managing patients in the immediate and long-term after transplantation, the incidence of graft dys¬function has also been considered. The issues of mortality, the survival of recipients and transplants are covered. Further ways for the development of a liver transplant program in the region are outlined.

ONCOLOGICAL ALERTNESS OF DENTISTS ACCORDING TO THE SURVEY IN THE MOSCOW REGION

Annotation:

Cancer incidence with the localization of the tumor in the maxillofacial region increases due to the deterioration of the environmental situation, the prevalence of bad habits, untimely access to the dentist for the treatment of dental diseases and preventive examinations, the complexity of diagnosis and treat¬ment of precancerous diseases of the oral cavity. It is necessary to activate the early diagnosis of cancer at the dental reception and the qualification of dentists of all specialties in matters of cancer alertness. There was executed a survey of 176 dentists in 12 districts of the Moscow region on 10 issues, reflect¬ing the frequency of detection of cancer pathology and precancerous conditions, doctors' knowledge of diagnostic methods and therapeutic tactics in the detection of this pathology. Most of the interviewed dentists of the Moscow region consider the full collection of anamnesis and examination of the maxil¬lofacial area and oral cavity in patients. Not more than 10% of dentists (mainly surgeons) reported about the treatment of patients with cancer (precancerous and cancerous diseases), at the same time about 80% of doctors met cancer in their work. Among the main diagnoses of cancer pathology, doc¬tors note cancer of the mucous membrane of the mouth, tongue, and lips; followed by leukoplakia and lichen planus. More than 40% of dentists do not use clinical and laboratory methods for the diagnosis of diseases of the mucous membrane of the mouth and lips; the most common method of diagnosis of diseases of the mucous is cytological and clinical examination. About 80% of dentists in the Moscow region had experience in referral cancer pathology patients for further examination and treatment in approximately equal proportion to dentists, surgeons, oncologists in the area and in M.F. Vladimirsky Moscow Regional Research Clinical Institute (less often MOS); dentists in a number of districts of the Moscow region do not use the opportunities of regional dental institutions. Half of the dentists do not fill in the reporting and regulatory documentation in the detection of cancer of the mouth. With a high readiness of more than 85% of dentists to use a new method of diagnosing cancer, about 40% of doctors were not familiar with the photodynamic screening test Vizi Lite Plus. The vast majority of dentists in the Moscow region consider it appropriate for the early diagnosis of cancer pathology organization of den¬tal examination, dental preventive examinations, as well as the use of screening tests, although a third of doctors did not give proposals to improve the early diagnosis of cancer. There is no doubt that there is a need to improve the organizational and diagnostic base of early diagnosis of cancer in dentistry.

THE EFFECT OF REHABILITATION MEASURES ON THE ADAPTATION POSSIBILITIES OF THE ORGANISM IN MILITARY EMPLOYEES SUFFERED FROM EROSIVE AND ULCEROUS DEFECTS OF THE STOMACH AND DUODENUM

Annotation:

The purpose of the study: the development of the innovative technology of medical rehabilitation (MR) of military personnel with erosive and ulcerative lesions of the stomach and duodenum (EULSD). Material and methods. The study included 154 military personnel cases divided into 4 randomized groups. Patients of the main group (MG) including 42 people, received standard anti-ulcer drug therapy (AUDT), endo-ecological rehabilitation (EER, levinafit and bottled drinking mineral water (MW)), and dynamic electroneurostimulation (DENS). In the 1st comparison group (CG) consisted of 38 people, patients received a standard AUDT and EER; in the 2 CG (38 people), patients received a standard AUDT and DENS using the same regimens as in MG. In the control group (CG), 36 patients received only standard PMTT. Results. Comparative analysis of direct and remote observations indicates the feasibility of the rehabilitation measures. Thus, the use of EER and DENS methods as an additional anti-relapse helps to consolidate the remission of erosive and ulcerative lesions of the stomach and duodenum in military personnel: the preservation of the achieved results of comprehensive medical rehabilitation of military men with erosive and ulcerative lesions of the stomach and duodenal ulcer using the methods of EER and DENS in the background of the PMR more than 9–12 months was noted in 86.7% of patients; in the comparison groups with isolated application of the EER or DENS methods against the background of the AUDT, the duration of remission for more than 9–12 months was established in 66.7 and 70.0% of the observed, respectively; when using only AUDT, only in 45.3% of military personnel. Conclusion. The introduction of EER methods (phytopreparations and drinking MV) and DENS in the standard treatment complex of AUDT in servicemen with erosive and ulcerative diseases of the stomach and duodenum is appropriate, as evidenced by a significant improvement in their quality of life.

Annotation:

There was executed an analysis of methods and results of studying the conditions and factors influencing now on a state of health of medical workers, including women, with various experience. Legal and regulatory documents regulating the assessment of conditions and factors affecting the health of health workers, especially women, have been systematized and analyzed. The methodology of an assessment of a set of conditions and factors affecting health workers, including women, in their workplaces is described. There considered causes and features of the development of professionally-related diseases of medical workers, including women, different specialties and different ages. Recommendations on prevention and minimization of occupational morbidity of medical workers, including women, working in the health system of the megalopolis are formed.

ANALYSIS OF 57 YEARS MORTALITY FROM MALIGNANT NEOPLASMAS IN A POPULATION COHORT, IRRADIATED IN THE EAST-URAL RADIOACTIVE THACE

Annotation:

Introduction. The explosion occurred in September 1957 in the Mayak radioactive waste storage facility led to the formation of the East-Ural Radioactive Trace (EURT) and the exposure of the population living in this territory. The aim of the work is to present an analysis of mortality from malignant neoplasms among cohort members irradiated on the EURT territory depending on demographic characteristics over the observation period, extended by 7 years compared with previous studies. Material and methods. In the nineties, in the Ural Scientific and Practical Center of Radiation Medicine, a cohort of the population irradiated at EURT was formed, the number of which in recent years has been expanded to 21.4 thousand people, and the observation period has been extended to 57 years. The paper describes the demographic characteristics of the EURT population cohort, the structure of mortality from malignant neoplasms (1,429 cases in the observation area) and an analysis of the patterns of changes in mortality in cohort members from 1957-2014 relating to the gender, age, nationality, and calendar period. The mortality rates presented in the paper are calculated per 100,000 person-years of the period under consideration, the confidence intervals were determined with a 95% probability based on the Poisson distribution. The results of the analysis of mortality in East-Ural radioactive trail cohort for 1957-2014 do not contradict previous studies and correspond to the general trends in the dependence of cancer mortality related to the gender, age and calendar period observed in the Russian Federation, but they have their own characteristics in the first years after the accident.

Annotation:

The article presents the results of retrospective data analysis about the morbidity of population of the Tosnen¬sky district in the Leningrad region, living in the area of location of the Polygon «Krasny Bor» according to Territorial Compulsory Medical Insurance Fund over a three year period. During the study, a children health status was assessed because this group is most affected by chemical factors. In the study, there was used the method of converting cases of medical care provided to the children into generally accepted incidence rates of the population with the subsequent transfer of personalized databases to the data warehouse of the police records and the performance of the epidemiological analysis. As a result of a retrospective analysis of a level and structure of the incidence of children in the Tosnensky district, there is no reason to accept the hypothesis of an increased risk of morbidity among children living near the Polygon «Krasny Bor».

Annotation:

The methodology has been worked out and equipment has been selected for the small-dose application of microbial suspension to the soil using bacteria and yeast as an example. The survival rate of microorganisms-destructors under the action of solar radiation was studied when they were metered into the soil and on the walls of buildings. Field tests of biotechnology have been carried out (using the small-dose introduction of destructor microorganisms) for in situ rehabilitation of the territory contaminated with polychlorinated biphenyls and diesel fuel. During bioremediation, a decrease in the integral toxicity and phytotoxicity of the soil being cleaned, an increase in its biological activity (the level of dehydrogenases, hydrolases, the intensity of cellulose decomposition) was revealed. The use of microencapsulated microorganisms-destructors reduces the stress from entering into contaminated toxic soil, increases their activity, enhances the effectiveness of remediation. The necessary regulatory and technical documentation on the bioremediation of areas contaminated with toxic chemicals and POPs has been developed.

Functional status of personnel was monitored at chemical weapons destruction facilities in order to timely identify early signs of exposure of harmful and dangerous production factors, as well as a number of somatic diseases (primarily cardiovascular), which are common contraindications for working with toxic substances. The analysis of the prevalence of risk factors of the development of cardiovascular pathology is carried out. Persons with a high and moderate risk of cardiovascular diseases were identified.

APPLICATION OF BIOFEEDBACK METHOD IN COMPLEX SANATORIUM-RESORT REHABILITATION OF NUCLEAR INDUSTRY WORKERS WITH ARTERIAL HYPERTENSION AND CHRONIC PSYCHO-EMOTIONAL STRESS

Annotation:

There was performed a survey of 66 male employees of the Siberian chemical plant (SCP) exposed to ionizing radiation and suffered from arterial hypertension and psycho-emotional stress, the average age accunted for 48.3 ± 8.4 years. The analysis of blood pressure and the study of psychological status were based on questionnaires Spielberger - Khanin, SF-36, PSM-25. As an assessment of the presence of psycho-emotional stress and chronic fatigue syndrome, there was performed a test "MOPSY" to assess the degree of exhaustion of vitality and the threat of depression. The elevation in blood pressure was found in 78% of the surveyed SCP workers at the same time, the average values of systolic blood pressure were of 145.16 ± 4.3 mm Hg, diastolic blood pressure - 90 ± 0,57 mm Hg. Signs of exhaustion test "MOPSY" have been found in all examined workers. Increased levels psycho-emotional stress according to the PSM-25 questionnaire were found in 82% and 74% of patients, respectively. Against the background of complex sanatorium-resort rehabilitation, it was possible to achieve the target level of blood pressure in all the studied groups. Correction of indices of psychological status was more pronounced when biofeedback training was included in the treatment. The frequency of registration of a high level of life exhaustion and intense stress load on the background of biofeedback training decreased by 80%, while only in the group of biofeedback training in some patients (14 (30.43%) people) completely stopped the signs of stress load and life exhaustion and they moved to the group with no signs of overstrain and fatigue (p < 0.001, χ2 = 16.722). Against the background of biofeedback training the level of stress significantly decreased (from 16,6 [8;18] to 8 [4;13] points, p = 0,0001...), psycho-emotional tension (from 22 [18;24] to 14 [12;18] points, p = 0,0001...) and situational anxiety (from 59 [45;68] to 50,5 [43,5;60,5] points, p = 0,0001...), respectively, increased the performance of the nervous system. Patients have become more appreciative of their subjective well-being and quality of life, and as a "physical component of health" (physical functioning; role functioning and General health) and the "psychological component of health" in the SF-36 questionnaire.

THERMAL STATE OF COSMONAUTS UNDER AUTOMATIC CONTROL OF HEAT REMOVAL DURING EXTRAVEHICULAR ACTIVITY ON INTERNATIONAL SPACE STATION

Annotation:

The paper is devoted to the presentation and analysis of the main results of medical monitoring for health and thermal state of two Russian crewmembers of the International Space Station (ISS) during their first two extravehicular activity (EVA) in the Orlan-MKS spacesuit equipped with an automatic thermal control system (ATCS). The duration of the first EVA accounted for 7 h 34 min, of the second one - 8 h 12 min. ATCS began to function after the transfer to the autonomous mode of the spacesuit and the inclusion of sublimation heat exchanger. To maintain the thermal balance of cosmonaut working in a spacesuit, a positive relationship was used between the value of a body heat production and the level of CO2 eliminated during the respiration. Metabolic rate was used as a control signal of the ATCS. In the whole range of metabolic rates from 1.5–2 kcal/min in relative rest to 5–7 kcal/min at maximum physical loads, the body temperature of the EVA participants remained at a normal level within 36.3-37.0°C. The heart rate changed from 61-64 beats/min at rest to 133-136 beats/min in the period of the most complex operations with the heavy physical load. Based on medical monitoring data the cosmonaut’s thermal status during spacewalks was estimated as a close to comfortable. The results of maintaining the thermal balance of cosmonauts by means of a manual method for regulating the heat removal during the EVAs, performed onboard the Salyut-6, Salyut-7 and Mir orbital stations, as well as the ISS, are discussed. The use of the Orlan-MKS spacesuit with ATCS allowed for automatic control over the maintenance of a close to a comfortable thermal state of the body without distracting the cosmonaut’s attention from performing the main EVA tasks increasing significantly a safety during a humans’ work in the open space.

THE STUDY OF THE MEDICO-BIOLOGICAL EFFECTS CREATED BY THE SYSTEM OF ACTIVE HYDROACOUSTIC PROTECTION "ZEVS" OF THE NEW GENERATION

Introduction. The currently being developed means for the protection of objects of the water area from divers-violators require medical and biological testing on laboratory animals to assess the effectiveness and safety of the use of these tools. One of such means of protection of objects from the water area is the active hydroacoustic protection system (HAS) with the non-lethal action “ZEVS” of a new generation. The aim of the work is to substantiate the recommendations for the safe use of the HAS ZEVS of new generation using biomedical experimental studies on laboratory animals (rabbits). Material and methods. The peak pressure of the sonar pulse was 405, 880 and 1124 kPa. The pulse du¬ration accounts for approximately 0.1 ms. Scheme of medical and biological tests. A device for rabbits breathing underwater has been manufactured and put into operation. In total, six series of experiments were carried out on six groups of rabbits, formed depending on the peak pressure of the sonar pulse, the location of the rabbit in respect of the radiator and the multiplicity of exposure. The tests were carried out in a filled with water pool having the shape of a parallelepiped with a length of 4, a width of 2 and a height of 2 m with sound-absorbing walls. Evaluation of the effect on animals of factors generated by HAS ZEVS was carried out in accordance with the standards of the A.I. Burnazyan Federal Medical Biophysical Center. Results. Analysis of HAS ZEVS effects on the central nervous system of rabbits by the method of acoustic stem evoked potentials Analysis of the evoked potentials of the rabbits’ brain led to the conclusion that there was no damaging effect of the HAS ZEVS on the central nervous system and the auditory analyzer. However, the action of a hydroacoustic pulse with a peak pressure of 1124 kPa can cause a state of mild stupor, which, in combination with factors of the underwater environment, represents a threat to life. Analysis of HAS ZEVS effects in the pathological examination of rabbits The results of the clinical exami¬nation and postmortem examination of rabbits after exposure have led to the conclusion that as a result of the HAS ZEVS impact, no significant visible pathological changes in internal organs of biological objects and, in particular, from critical organs have been identified. Analysis of the effects of HAS ZEVS on the biophysical parameters of the lungs The results of measurements of biophysical indices of the lungs indicate that out of 45 rabbits of the experimental group, one was found to have a slight severity. Conclusion. The result of the work done is summarized, and the recommendations on the safe use of the new generation HAS ZEVS are substantiated.

Aim of the study. Screening diagnostics of the athletes-divers’ functional state with a predominance of autonomic type of regulation. Materials and methods. Measurement of the heart rate variability was carried with the use of "Diamed- MBS" hardware and software complex according to the guidelines of the State Research Center – A.I. Burnasyan Federal Medical Biophysical Center of Federal Medical-Biological Agency, Moscow, 123098, Russian Federation. Received data were analyzed with a specialized application package of statistical data processing "KNIME" and "Statistica 7". Results Linear discriminant analysis showed high classification ability of differential diagnosis of functional state level in athletes-divers’ providing a predictive match in 91% of cases with real results. Received data prove a proposed model of the diagnostics of the athletes’ functional state level to ensure coincidence of predictable diagnosis with the real result in 80% of LDF1, 100% of LDF2, 100% of LDF3, 81,8% of LDF4, 75% of LDF5, 96,3% of LDF6, 91,8% of LDF7 and 91.4% of LDF8. Conclusions. 1. The analysis of results of obtained data of heart rate variability’s method identifies the main features giving the ability to diagnose the functional state level of the organism in sportsmendivers with a predominance of autonomic type of regulation. This results allow predicting the health level based on the the following characteristics: Si, Mx, AMo50, IPRS, Mn, PARS, LF, CV, MxRMn, HR, Mo, RMSSD, pNN50, CC1, R-R intervals, HFt, LF/HF, VLF/HF, LF, AMoSDNN, SDNN, VLF. 2. There has been generated statistically significant (p<0.001) mathematical model of prognosis of functional state level in athletes-divers' using the obtained features. 3. Highly informative linear discriminant function is designed. Such a function is capable to determine the functional state level of sportsmen-divers’ organism with a predominance of autonomic type of regulation in a short time with a probability of 91%.
